Who we are
Connected care from Unity Health Network
Healthcare is better when care is connected. As one of Northeast Ohio's largest physician networks, Unity Health Network brings together primary care physicians and specialists to provide coordinated, patient-centered care.
Our providers work across communities and care settings to help patients access the expertise, support and seamless experience they need to achieve better health at every stage of life.
Our history
A physician led network built to strengthen independent care
Unity Health Network was founded in 2002 on the belief that independent physicians could achieve more together than they could alone. Led by Dr. Robert Kent, a small group of physicians created a collaborative network designed to preserve independence while improving efficiency, support services and patient care.
What began with three physicians and their staff has grown into a thriving network supported by more than 400 healthcare professionals who share a commitment to delivering high-quality care throughout Northeast Ohio.
Our approach to care
Leading with value-based care
Exceptional healthcare extends beyond treating illness. Unity Health Network takes a value-based care approach that emphasizes prevention, care coordination and better outcomes.
By connecting physicians, specialists, hospitals, skilled nursing facilities and home health providers, we help ensure patients receive the right care at the right time. This coordinated model has helped reduce avoidable hospitalizations, improve preventive care and support healthier communities across Northeast Ohio.
